While transcribing reports, please have your team follow the below-mentioned points.

 

1.     Type all reports, separated by page breaks with audio name at the start of document, in one Word File (saved as current DOS).

2.     Start typing just from Preop diagnosis till the End of report.

3.     No schedules to follow.

4.     No extra formatting.

5.     No date of dictation/DOS.

6.     No dictating doctor’s name at the beginning or at the end of the report.

7.     No referring/CC physician’s name. Write it just phonetically if it comes in the middle of the report.

8.     No double spacing.

9.     No extra headings except for preop/postop diagnosis, procedure, and surgeon’s name. (delete all other non-dictated headings.)

 

Daily line count will be calculated based on the MT version of reports. Kindly maintain a sheet of the same at your end with DOS, audio names, and line count of each report. :shy:
