You do not hav to get confused on this issue. Just make sure if u can use "a" before followup then it is a single word (there are exceptions!!!), Eg. a followup appointment; for a followup tomorrow; a scheduled followup appointment; a followup scheduled on Tuesday; for a followup next monday.

If "a" cannot be used, then it is a double word, follow up. for eg. the patient was asked to follow up next week, The patient was asked to follow up with an appointment (plz note "a" cannot be used here before follow up, so double word).

for your reference:

The patient is to follow up on Monday.
The patient is here for a followup.
The patient has been asked to follow up with Dr Dass.
The patient's followup has been scheduled for the next week.
The patient is here for a followup.
The patient has requested a followup appointment tomorrow.
